How to make a referral to the Beacon Program...

Referral Process

1.  When an IEP team first feels local programming is not creating positive behavioral outcomes, contact your special education coordinator and share with them your concerns.  Ideally, the team will gradually increase the student to Level 3 EBD services.   Once the team, including the parent, is ready to make an official referral to Beacon, contact Laurie Fevig for a referral packet.

2. Completed referral packet will include current EBD ER, FBA, PBSP, IEP, school records, agency contacts, diagnostic information along with a description of target behaviors and rationale for Beacon placement.  Send completed packet to Laurie Fevig at WCED.

3. The referral review team at WCED will review the file as soon as possible.   If the recommendation is for Beacon placement, the home district will assist Beacon case manager in setting up a change of placement IEP meeting, along with tour if necessary.

4.  Change of placement IEP meeting will include: overview of program, student’s specific needs and goals and setting up a meeting in 6 weeks for initial review.
